PURPOSE: Craigieburn Education and Community Centre Inc. (CEACC) is a non-profit organization, and is committed to meeting the requirements of the Information Privacy Act 2000 (Vic), the Health Records act 2001 (Vic) and the Disability Discrimination Act 1993.

SCOPE: This policy applies to all staff employed by or contracted to CEACC and all client information held within this organisation.
RESPONSIBILITY: The Committee of Management is responsible for the control and implementation of this policy, and will be made available to Service Users, Staff, Tutors, Committee of Management members, and any other concerned parties.
COLLECTION OF PRIVATE INFORMATION: Individuals from whom information might be obtained or requested could include Service Users, Staff, Tutors, Committee of Management, Centre User Groups, Facility Hirers, Casual Contacts, and Information Seekers. Information could be obtained in writing (forms, documents, etc), verbally (face to face, telephone, recording, etc), or electronically (facsimile, e-mail, etc). Only information that is needed or required to be collected is to be requested. This should be collected sensitively and discretely, and collected from that person or the person’s parent or guardian. The reason for collection needs to be advised to the person, or understood by the person.
USE AND DISCLOSURE: CEACC will use or disclose information only for the purpose that it is collected. Personal information shall not be disclosed without the person’s express consent. Information may be used in an aggregated form for secondary purposes such as planning. CEACC will seek consent (informed, freely given, specific and current) from individuals as required. This consent could for example be via statement and signature on an information form, explanation and acceptance, or request from the information provider.
DATA QUALITY: CEACC will endeavour to ensure the information is accurate and up to date.
DATA SECURITY AND RETENTION: CEACC will not allow information to be lost, modified, and disclosed or to be accessed without authority. We will manage information via proper process or systems and ensure that the information is kept in a secure environment.
ACCESS AND CORRECTION: Individuals have the right to access their own information. Information should be corrected where appropriate.
ANONYMITY: An individual has the right to deal with the service on an anonymous basis as long as this is lawful and it is reasonably practicable.
TRANSFER TO ANOTHER PARTY: CEACC will ensure that information is not to be transferred to other parties unless the transfer is the reason for which the data was collected and agreed to in the first instance, or the transfer is part of the normal (acceptable) process of providing the service required / requested by the individual. We will not transfer information to other parties unless the individual has provided consent to do so.
SENSITIVE INFORMATION: CEACC will ensure that sensitive information, such as race, religion, sexual preference, criminal record and membership of groups, is only collected with the individual’s consent.
